The CCG values working with a wide variety of partners including the Voluntary and Community Sector (VCS). A variety of voices and opinions help us to develop and implement our commissioning plans.

We are active member of Multi-Agency Groups across the county including:

  • Healthy Worcester
  • Malvern Health and Wellbeing Partnership
  • Wychavon Multi Agency Group
  • Wyre Forest Multi Agency Group


Healthwatch lead engagement on the NHS Long Term Plan in Spring 2019. STP partners co-produced the questions to ask patients and Healthwatch disseminated and analysed this, focusing on hard to reach communities. Their feedback report is available here

Social Prescribing Pilot

An example of working with the partners including the VCS is our recent Social Prescribing Pilot:

Social Prescribing is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services. 

The Worcestershire pilot model follows the model outlined in the 'Making Sense of Social Prescribing' document produced by the University of Westminster and endorsed by the Social Prescribing Network and NHS England. The Worcestershire programme aims to take a holistic, person-centred, community asset based approach, to delivering care, and promoting health and wellbeing.

The Social Prescriber acts as the 'bridge' between primary care/neighbourhood team professionals and the variety of opportunities locally; whether that is information, advice and guidance or signposting to local groups, organisations or services. Further information is available here

The VCS supported us in the design of the pilot, and in 2018-19 four organisations were commissioned as providers of the service. Approximately 800 'signposts' were made to organisations across Worcestershire. The pilot evaluation infographic gives further details

This has been evaluated and the finding of the pilot have informed the delivery of the new Social Prescribing model across Herefordshire and Worcestershire as part of the new GP contract (2019-20).
